Our office participates in Medicare, Blue Shield and most managed care organizations. Under your plan of coverage, you may be responsible for yearly deductibles or co-insurance payments.

If you are enrolled in a managed care program (HMO), you may need to contact your primary care physician for a referral prior to your appointment. Contact your insurance company regarding reimbursement policies. Any unpaid balance remains your responsibility.

Please bring your insurance cards and a photo I.D. to our office on the day of your first visit and keep us updated regarding any coverage changes.
