Our office participates in Medicare, Blue Shield and most managed care organizations. Under your plan of coverage, you may be responsible for yearly deductibles or co-insurance payments. If you are enrolled in a managed care program (HMO, PPO, POS, etc.), you may need to contact your primary care physician for authorization prior to your appointment. Contact your insurance company regarding reimbursement policies. Any unpaid balance remains your responsibility. Please bring your insurance cards to our office on the day of your first visit and keep us updated regarding any coverage changes.
